If you face any issue while embedding your forms then make sure to follow these steps below:
Make sure that your embed code is correctly copied and installed.
If you are using wordpress or an other builder, please create a new fresh page without any other stuff and embed the form then test how it looks like in full.
Once tested on the fresh new page then you can embed it on any page or footer.
If you use the button or link to trigger your form then After adding one of the codes, make sure that JavaScript snippet is included into your page
